What to bring to your appointment
Patients, please remember to bring the following items to your appointment:
- Current Referral (no Medicare rebate is applicable without a valid referral)
- Medicare Card
- Pension, Concession or DVA Card
- Private Health Insurance Details (If applicable)
- Recent medical imaging results
- Recent Blood Results
- Current Medications List
- Any previous correspondence
Referral
We accept all referrals from your GP or other specialists. There may be situations or issues where your referrers did not addressed the referral to us directly. We are happy to accept these referrals, which will save you another trip to your referring doctors.
